HERE'S HOW IT WORKS IF YOUR MEDICARE IS PRIMARY. . .

If you have Medicare Part B, you may be eligible for the following supplies from Diabetic Service Foundation.

• Glucose Test Strips
• Lancets
• Blood Glucose Monitor
• Lancing Device
• Control Solution

Medicare will pay 80% of the approved cost of these supplies after Diabetic Service Foundation submits the claim to Medicare on your behalf.  If you also carry Medicare supplemental coverage, the remaining 20% cost will be paid by the supplemental company.  If you don't have supplemental coverage, you are responsible for the remaining 20%.  All customers are responsible for any applicable deductibles.

Diabetic Service Foundation will:

• Treat you with prompt, courteous, and reliable service.
• Provide FREE DELIVERY of all supplies.
• Handle all paperwork and claim submission.
